I would like to be referred for an adult ADHD assessment. I have completed the ASRS-v1.1 self-screener and scored above the threshold. The symptoms have affected me since childhood and impact my work, relationships and daily functioning.
Under the Right to Choose framework in England, I would like to be referred to Psychiatry-UK or ADHD-360, which are NHS-contracted providers. I would like to discuss a referral under the NHS England Right to Choose framework, if I am eligible and an appropriate provider is available.
